摘 要
随着中国老龄化程度不断加深，社区老年人长期照护服务需求日益增长，但现有服务体系存在供给不足、专业化水平低等问题，社会工作介入成为提升服务质量的重要途径。本研究旨在探讨社会工作介入社区老年人长期照护服务的有效模式，以期为完善社区养老服务体系提供理论支持与实践指导。通过文献分析法梳理国内外相关研究成果，明确研究框架；运用案例分析法选取典型社区进行深入剖析，总结经验教训；借助问卷调查法收集大量一手数据，确保研究结论的科学性。研究发现，社会工作介入能够整合社区资源，构建多主体协同的服务网络，形成预防 - 干预 - 康复一体化的连续性照护体系。

Abstract
With the deepening of aging in China, the demand for long-term care services for the elderly in communities is growing, but the existing service system has the problems of insufficient supply and low professional level, and the intervention of social work has become an important way to improve the quality of service. The purpose of this study is to explore the effective mode of social work intervention in long-term care services for the elderly in communities, in order to provide theoretical support and practical guidance for improving the care of the community elderly service system. Organize relevant research results at home and abroad through literature analysis, clarify the research fr amework, select typical communities and analyze them deeply, and collect a large amount of primary data through questionnaire survey to ensure the scientific research conclusions. It is found that social work intervention can integrate community resources, build a multi-subject collaborative service network, and form a continuous care system integrating prevention, intervention and rehabilitation.
