摘 要
社区康复是提升残疾人生活质量、促进社会融合的重要途径，社会工作在其中发挥着不可替代的作用。本研究基于国内外社区康复理论与实践发展现状，聚焦中国国情下社区康复项目中社会工作的角色定位与实践模式。通过文献分析、案例研究和实地调研相结合的方法，选取具有代表性的社区康复项目进行深入剖析。研究发现，社会工作者在社区康复项目中扮演着资源整合者、服务协调者、倡导者和支持者的多重角色，不仅为康复对象提供直接的专业服务，还积极构建社区支持网络，推动政策倡导。创新性地提出“全人康复”理念指导下的社区康复服务框架，强调从生理、心理、社会三个维度全面提升康复对象的生活质量。该框架突破了传统单一的康复模式，将家庭、社区资源有效整合，形成多主体协同参与的服务体系。研究结果表明，社会工作介入能够显著提高社区康复项目的有效性与可持续性，为完善我国社区康复服务体系提供了重要的理论依据和实践参考。

Abstract
Community-based rehabilitation is a critical pathway for enhancing the quality of life and promoting social integration of people with disabilities, where social work plays an irreplaceable role. This study examines the role positioning and practice models of social work in community rehabilitation projects under the context of China’s national conditions, based on the current development of community rehabilitation theories and practices both domestically and internationally. By employing a combination of literature analysis, case studies, and field research, this study conducts an in-depth analysis of representative community rehabilitation projects. The findings reveal that social workers assume multiple roles as resource integrators, service coordinators, advocates, and supporters within community rehabilitation projects. They not only provide direct professional services to rehabilitation clients but also actively construct community support networks and promote policy advocacy. Innovatively, this study proposes a community rehabilitation service fr amework guided by the concept of "holistic rehabilitation," which emphasizes improving the quality of life of rehabilitation clients from physiological, psychological, and social dimensions. This fr amework transcends traditional single-focused rehabilitation models by effectively integrating family and community resources, forming a multi-stakeholder collaborative service system. The results indicate that the involvement of social work significantly enhances the effectiveness and sustainability of community rehabilitation projects, providing important theoretical foundations and practical references for improving China’s community rehabilitation service system.
