摘 要
随着我国社会经济的快速发展，社区残疾人康复服务成为提升残疾人生活质量、促进社会融合的重要途径。本研究旨在探讨社会工作在社区残疾人康复服务中的作用与实践模式，通过文献综述和案例分析相结合的方法，选取具有代表性的社区进行深入调研。研究发现，社会工作者凭借专业优势，在需求评估、资源整合、服务协调等方面发挥着不可替代的作用，能够为残疾人提供个性化、连续性的康复支持。创新性地提出“全人康复”理念，将生理康复与心理社会支持有机结合，构建了多学科协作的服务体系。研究结果表明，社会工作介入显著提高了康复服务的有效性和可及性，改善了残疾人的生活状态和社会参与度。

Abstract
With the rapid development of China's social economy, community rehabilitation services for the disabled have become an important way to improve the quality of life of the disabled and promote social integration. This study aims to explore the role and practice patterns of social work in community rehabilitation services for disabled persons, and to select representative communities for in-depth research through a combination of literature review and case analysis. The study found that social workers play an irreplaceable role in demand assessment, resource integration and service coordination with their professional advantages, and can provide personalized and continuous rehabilitation support for the disabled. It innovatively puts forward the concept of "whole-person rehabilitation", organically combines physical rehabilitation and psychosocial support, and constructs a multi-disciplinary cooperative service system. The results show that social work intervention significantly improves the effectiveness and accessibility of rehabilitation services, and improved the living conditions and social participation of persons with disabilities.
