摘 要
随着中国人口老龄化进程的加速，社区养老成为应对老龄化社会的重要模式，然而传统社区养老面临服务内容单一、专业人才匮乏等问题，为改善这一状况，本研究旨在探讨社会工作介入社区养老模式的实践路径与创新策略。以北京市某社区为例，采用文献研究法梳理国内外相关理论成果，运用问卷调查和访谈法收集一手数据，对社区内老年人需求进行全面评估。结果显示，社会工作介入后，通过建立个案管理模式、组建志愿者团队、开展丰富多彩的文化娱乐活动等措施，显著提升了老年人的生活质量和社会参与度，有效缓解了家庭养老压力。创新点在于构建了“社工+志愿者”联动服务体系，实现了养老服务供给的专业化与多元化；开发了智慧养老信息平台，整合各类资源，提高了服务效率。

Abstract
With the acceleration of the aging process of China's population, community pension has become an important mode to deal with the aging society. However, the traditional community pension is faced with problems such as single service content and lack of professional talents. In order to improve this situation, this study aims to explore the practical path and innovative strategy of social work intervention in community pension model. Taking a community in Beijing as an example, the literature research method was used to sort out the relevant theoretical achievements at home and abroad, and the questionnaire survey and interview method were used to collect first-hand data to conduct a comprehensive assessment of the needs of the elderly in the community. The results show that after the intervention of social work, the quality of life and social participation of the elderly were significantly improved through the establishment of case management mode, volunteer teams and the development of colorful cultural and recreational activities, and the pressure of family pension was effectively alleviated. The innovation lies in the construction of "social worker + volunteers" linkage service system, realizes the specialization and diversification of pension service supply, develops the intelligent pension information platform, integrates various resources, and improves the service efficiency.

Abstract
