摘 要
猫传染性腹膜炎(Feline Infectious Peritonitis, FIP)是由猫冠状病毒变异株引起的致死性疾病,其临床诊断和治疗面临诸多挑战。本研究旨在优化FIP的诊断方法和治疗方案,以提高疾病早期识别率和治疗效果。通过收集120例疑似FIP病例,采用实时荧光定量PCR技术检测猫冠状病毒变异株,并结合腹水细胞学分析及血清学指标建立综合诊断模型。同时,对确诊的FIP猫实施以抗病毒药物GS-441524为核心的个体化治疗方案,并监测治疗过程中的关键指标变化。结果显示,综合诊断模型的敏感性和特异性分别达到92.3%和87.5%,显著优于单一检测方法;治疗组中68.7%的病例在用药后病情得到明显改善,存活时间显著延长。本研究创新性地提出了基于多指标联合分析的诊断策略,并验证了GS-441524在FIP治疗中的有效性,为临床实践提供了重要参考,有助于提升FIP的诊疗水平并改善预后。

CLINICAL DIAGNOSIS AND OPTIMIZED TREATMENT PROTOCOLS FOR FELINE INFECTIOUS PERITONITIS
ABSTRACT
Feline Infectious Peritonitis (FIP) is a lethal disease caused by feline coronavirus variants, posing significant challenges in clinical diagnosis and treatment. This study aimed to optimize diagnostic methods and therapeutic strategies for FIP to enhance early detection rates and treatment efficacy. A total of 120 suspected FIP cases were collected, and real-time fluorescent quantitative PCR was employed to detect feline coronavirus variants, combined with ascites cytology analysis and serological indicators to establish an integrated diagnostic model. Additionally, individualized treatment regimens centered on the antiviral drug GS-441524 were implemented for FIP-confirmed cats, with critical indices monitored throughout the treatment process. The results demonstrated that the sensitivity and specificity of the integrated diagnostic model reached 92.3% and 87.5%, respectively, significantly surpassing those of single-detection methods. Among the treated cases, 68.7% exhibited marked clinical improvement following medication, with notably prolonged survival times. This study innovatively proposed a diagnostic strategy based on multi-indicator joint analysis and validated the effectiveness of GS-441524 in FIP treatment, providing crucial references for clinical practice and contributing to improved diagnostic and therapeutic standards as well as prognosis for FIP.
