摘 要
慢性病已成为全球公共卫生领域的重大挑战，其长期性和复杂性对患者的自我护理能力提出了更高要求。本研究旨在探索提升慢性病患者自我护理能力的有效策略，以改善其健康状况和生活质量。研究采用混合方法设计，结合定量与定性分析，选取了300名慢性病患者作为研究对象，通过问卷调查、深度访谈及案例分析等方式收集数据。结果表明，患者的自我护理能力受多方面因素影响，包括疾病认知水平、社会支持系统以及心理适应能力等。基于此，研究提出了一系列创新性干预措施，如个性化健康教育方案、多层次社会支持网络构建以及心理韧性培养计划。这些策略在实践中显著提升了患者的自我护理能力和健康管理水平。本研究的主要贡献在于将理论框架与实际应用相结合，为慢性病管理提供了可操作性强的干预模式，并强调了跨学科协作的重要性，为未来相关研究奠定了基础。

Abstract
Chronic diseases have become a major challenge in global public health, with their long-term and complex nature imposing higher demands on patients' self-care abilities. This study aims to explore effective strategies for enhancing the self-care capabilities of patients with chronic diseases to improve their health conditions and quality of life. A mixed-methods design was employed, integrating quantitative and qualitative analyses, with 300 patients with chronic diseases selected as research participants. Data were collected through questionnaires, in-depth interviews, and case analyses. The findings indicate that patients' self-care abilities are influenced by multiple factors, including disease awareness levels, social support systems, and psychological adaptability. Based on these insights, the study proposes a series of innovative interventions, such as personalized health education programs, the construction of multi-level social support networks, and resilience-building initiatives. These strategies have significantly improved patients' self-care abilities and health management skills in practice. The primary contribution of this study lies in its integration of theoretical fr ameworks with practical applications, providing an actionable intervention model for chronic disease management and emphasizing the importance of interdisciplinary collaboration, thereby laying a foundation for future related research.
