  随着全球人口老龄化加剧,老年患者用药安全成为亟待解决的重要公共卫生问题。本研究旨在系统探讨老年人药物代谢特点及其对临床合理用药的影响,通过文献综述与临床数据分析相结合的方法,全面梳理了老年患者生理机能变化对药物吸收、分布、代谢和排泄的影响机制。研究发现,老年人肝肾功能减退导致药物清除率下降,脂肪组织增加影响亲脂性药物分布容积,细胞色素P450酶系活性降低改变药物代谢速率,这些因素共同作用使得老年患者更容易发生药物蓄积及不良反应。基于上述研究结果,本文提出了个体化给药方案设计原则,强调应根据患者具体健康状况调整初始剂量并密切监测血药浓度,同时建议优先选用经过充分老年安全性评价的药物品种。本研究创新性地构建了涵盖药动学参数修正、合并用药风险评估及药物相互作用预警在内的综合用药指导体系,为提高老年患者用药合理性提供了科学依据,对优化临床治疗策略具有重要参考价值。

Abstract

  As the global population ages, medication safety in elderly patients has become an urgent public health issue. This study aims to systematically investigate the characteristics of drug me tabolism in elderly individuals and their impact on rational clinical medication use. By integrating literature review with clinical data analysis, this research comprehensively examines how physiological changes in elderly patients affect drug absorption, distribution, me tabolism, and excretion. The findings indicate that diminished hepatic and renal function in the elderly leads to reduced drug clearance rates, increased fat tissue alters the volume of distribution for lipophilic drugs, and decreased activity of cytochrome P450 enzymes modifies drug me tabolism rates. These factors collectively increase the susceptibility of elderly patients to drug accumulation and adverse reactions. Based on these results, principles for personalized dosing regimens are proposed, emphasizing the adjustment of initial doses according to individual health conditions and close monitoring of serum drug concentrations. It is also recommended to prioritize medications that have undergone thorough safety evaluations in elderly populations. Innovatively, this study constructs a comprehensive medication guidance system that includes pharmacokinetic parameter adjustments, polypharmacy risk assessments, and drug interaction alerts, providing a scientific basis for enhancing the rationality of medication use in elderly patients and offering significant reference value for optimizing clinical treatment strategies.
