摘 要
随着人口老龄化加剧和口腔健康意识提升，口腔种植术作为修复缺失牙齿的重要手段，其成功率及影响因素成为研究热点。本研究旨在系统分析口腔种植术的成功率及其关键影响因素，为临床实践提供科学依据。通过对2018 - 2022年期间国内15家三甲医院的3276例口腔种植病例进行回顾性分析，采用多因素Logistic回归模型探讨患者年龄、性别、吸烟史、骨质状况、种植体类型、手术技术等变量与种植成功率之间的关系。结果显示，总体成功率为94.3%，其中女性患者(95.6%)略高于男性(93.1%)，非吸烟者(95.8%)显著优于吸烟者(89.7%)；骨量充足组(96.5%)明显好于骨量不足组(87.2%)；ITI系统种植体(95.9%)表现最佳。创新点在于首次将患者心理状态纳入分析范畴，发现焦虑抑郁情绪会降低12.8%的成功率。此外，本研究构建了基于机器学习的预测模型，可提前准确预测种植失败风险，为个性化治疗方案制定提供重要参考。结论是口腔种植术具有较高成功率，但受多种因素综合影响，需针对不同患者特点采取个体化诊疗策略以提高成功率。

Abstract
With the intensification of population aging and enhanced awareness of oral health, dental implant surgery has become an important method for restoring missing teeth, making its success rate and influencing factors a research hotspot. This study aims to systematically analyze the success rate of dental implant surgery and its key influencing factors to provide a scientific basis for clinical practice. A retrospective analysis was conducted on 3,276 cases from 15 tertiary hospitals in China between 2018 and 2022, employing a multivariate logistic regression model to investigate the relationship between success rates and variables such as patient age, gender, smoking history, bone quality, implant type, and surgical technique. The results showed an overall success rate of 94.3%, with female patients (95.6%) slightly higher than males (93.1%), non-smokers (95.8%) significantly better than smokers (89.7%), adequate bone volume group (96.5%) markedly superior to insufficient bone volume group (87.2%), and ITI system implants performing best at 95.9%. An innovation of this study lies in incorporating patient psychological state into the analysis, revealing that anxiety and depression can reduce the success rate by 12.8%. Additionally, a predictive model based on machine learning was constructed, capable of accurately predicting the risk of implant failure in advance, providing crucial reference for personalized treatment planning. The conclusion is that dental implant surgery has a high success rate but is influenced by multiple factors, necessitating individualized diagnostic and therapeutic strategies tailored to different patient characteristics to enhance success rates.
