抑郁症是一种复杂的神经精神疾病,其病理机制尚未完全明确,而传统抗抑郁药物存在起效慢、副作用多等问题,因此探索新型干预手段具有重要意义。本研究旨在探讨针灸刺激特定穴位对抑郁症患者脑功能的调节作用及其潜在机制。研究选取了60名符合诊断标准的抑郁症患者,随机分为针灸组和对照组,针灸组接受为期8周的特定穴位(如百会穴、神门穴)刺激治疗,对照组则接受常规护理。通过功能性磁共振成像(fMRI)技术评估两组患者治疗前后静息态脑功能连接的变化,并结合汉密尔顿抑郁量表(HAMD)评分分析临床疗效。结果显示,针灸组患者的HAMD评分显著降低,且与对照组相比,其默认模式网络(DMN)的功能连接异常得到明显改善,特别是在前扣带回皮层和内侧前额叶皮层之间的连接强度趋于正常化。此外,针灸刺激还促进了左侧岛叶与边缘系统的功能耦合,这可能与情绪调节能力的恢复密切相关。本研究创新性地揭示了针灸通过调节特定脑区功能连接从而缓解抑郁症状的作用机制,为针灸治疗抑郁症提供了科学依据,并拓展了非药物干预在神经精神疾病领域的应用前景。

Abstract

Depression is a complex neuropsychiatric disorder with its pathological mechanisms yet to be fully elucidated. Traditional antidepressant medications are often associated with issues such as delayed onset of action and numerous side effects, highlighting the significance of exploring novel intervention strategies. This study aimed to investigate the modulatory effects of acupuncture stimulation at specific acupoints on brain function in patients with depression and its underlying mechanisms. Sixty patients diagnosed with depression were recruited and randomly assigned to either an acupuncture group or a control group. The acupuncture group received an 8-week treatment targeting specific acupoints, such as Baihui (GV20) and Shenmen (HT7), while the control group underwent routine care. Functional changes in resting-state brain functional connectivity before and after treatment were assessed using functional magnetic resonance imaging (fMRI), and clinical efficacy was analyzed based on Hamilton Depression Rating Scale (HAMD) scores. Results indicated that HAMD scores significantly decreased in the acupuncture group. Compared with the control group, the abnormal functional connectivity within the default mode network (DMN) was markedly improved, particularly in terms of normalized connectivity strength between the anterior cingulate cortex and the medial prefrontal cortex. Additionally, acupuncture stimulation facilitated functional coupling between the left insula and the limbic system, which may be closely related to the restoration of emotion regulation capacity. This study innovatively revealed the mechanism by which acupuncture alleviates depressive symptoms through the modulation of functional connectivity in specific brain regions, providing scientific evidence for the application of acupuncture in treating depression and expanding the potential of non-pharmacological interventions in the field of neuropsychiatric disorders.
