摘 要
中药与西药联合使用在临床实践中日益普遍，但其对肝功能的影响机制尚不完全明确。本研究旨在探讨中西药联用对肝脏代谢酶、氧化应激及炎症反应的作用机制，以期为临床安全用药提供理论依据。通过建立小鼠肝损伤模型并采用高效液相色谱法检测药物代谢产物，同时结合转录组学分析关键基因表达变化，发现中西药联用可通过调节CYP450酶活性和Nrf2信号通路减轻药物性肝损伤。此外，实验结果表明某些中药成分可拮抗西药引起的氧化应激反应，并抑制促炎因子的过度释放。本研究创新性地揭示了中西药联用在肝脏保护中的协同作用机制，为优化联合用药方案提供了新思路，同时强调了个体化用药的重要性，以降低潜在的肝毒性风险。关键词：中西药联用; 药物性肝损伤; CYP450酶; Nrf2信号通路; 氧化应激反应
Abstract
The combined use of traditional Chinese medicine (TCM) and Western medicine is becoming increasingly common in clinical practice; however, the underlying mechanisms of its effects on liver function remain incompletely understood. This study aimed to investigate the mechanisms by which the combination of TCM and Western medicine influences hepatic me tabolic enzymes, oxidative stress, and inflammatory responses, thereby providing a theoretical basis for safe clinical medication. A mouse model of liver injury was established, and high-performance liquid chromatography was employed to detect drug me tabolites, while transc riptomic analysis was used to examine changes in key gene ex pressions. The findings revealed that the combination of TCM and Western medicine could alleviate drug-induced liver injury by modulating the activity of CYP450 enzymes and activating the Nrf2 signaling pathway. Additionally, experimental results indicated that certain TCM components could counteract the oxidative stress response induced by Western medicine and inhibit the overproduction of pro-inflammatory factors. This study innovatively elucidated the synergistic mechanisms of TCM and Western medicine in liver protection, offering new insights into optimizing combination therapy regimens and emphasizing the importance of personalized medication to minimize potential hepatotoxic risks.
