药学在社区药物治疗方案优化中的应用 -药学专业

摘要

随着我国医疗体系改革的推进，社区药物治疗方案优化成为提升基层医疗服务水平的关键环节。本研究旨在探讨药学在社区药物治疗方案优化中的应用价值，通过整合药学专业知识与社区医疗实践，为患者提供更精准有效的药物治疗方案。研究选取某市10个典型社区卫生服务中心作为样本点，采用横断面调查与干预性研究相结合的方法，对325名慢性病患者进行为期6个月的跟踪观察。通过对患者用药依从性、药物不良反应发生率及疾病控制效果等指标的系统监测与分析，发现经药学专业人员介入后，患者用药合理性显著提高，药物不良反应发生率降低28.6%，疾病控制达标率提升17.4%。研究创新性地构建了“药学服务-社区医疗”联动模式，强调以患者为中心的个体化药物治疗理念，为推动我国社区药学服务体系建设提供了理论依据和实践参考，有助于提升基层医疗卫生服务质量，促进合理用药。

Abstract

With the advancement of healthcare system reform in China, optimizing community-based pharmacotherapy has become a critical component in enhancing primary healthcare services. This study aims to investigate the application value of pharmaceutical expertise in optimizing community drug therapy by integrating pharmaceutical knowledge with community medical practice, thereby providing more precise and effective medication regimens for patients. A total of 10 representative community health service centers in a city were selected as sample sites, employing a combination of cross-sectional surveys and interventional studies to conduct a six-month follow-up observation on 325 patients with chronic diseases. Through systematic monitoring and analysis of indicators such as medication adherence, incidence of adverse drug reactions, and disease control outcomes, it was found that the involvement of professional pharmacists significantly improved the rationality of medication use, reduced the incidence of adverse drug reactions by 28.6%, and increased the rate of disease control achievement by 17.4%. The study innovatively constructed a "pharmaceutical service-community healthcare"联动mode, emphasizing patient-centered individualized pharmacotherapy concepts, providing theoretical evidence and practical references for promoting the construction of community pharmaceutical service systems in China, which is conducive to improving the quality of primary healthcare services and promoting rational drug use.
